Chronic Disease Management

Our CDM allied health consultations are aimed at improving the quality of life for people with chronic disease and complex illness. Conclusive evidence shows that structured physical activity and collaborative lifestyle programs are an integral therapy in the management of various chronic conditions.

Team Care Arrangements

The Vario Wellness clinic welcome invites to be part of Management Care Plans and have a full team of allied health professionals who work collaboratively to ensure best possible patient outcomes.

Our philosophy is to support the patient to better manage themselves to ensure that their physical activity and lifestyle changes are sustainable long-term. Our consultations are based on the principles of self-management, incorporating motivational interviewing, working in collaboration with the client to design the optimal program, and developing self-efficacy through progressive overload and specific goal-setting.
